The Partners Program, offered in partnership with Lutherwood, provides intensive home-based support services to families within Waterloo Region with children and youth, 7 to 18 years of age, who are experiencing or at risk of developing social, emotional, behavioural and/or mental health challenges.
A Family Support Worker will work with your family in your home or in the community for up to three months to help increase your understanding of your unique family, reinforce family strengths, build coping skills and enhance the capacity of parents and youth to function both as a family unit and as part of the community. The Family Support Worker engages you, the family, as equal partners to identify desired changes, set goals, develop skills and practice strategies in order to address challenges. Families are involved in tracking progress and evaluating success throughout the program.
